Responsibilities

The Home Care Scheduler schedules home care, clinicians and patients in electronic medical record and assists supervisors in ensuring assignment of unscheduled visits. Performs secretarial and computer support tasks, acts as a receptionist in answering telephone for office, processes information of a confidential nature, and acts as a liaison between patient/family and home care staff.

Schedule: Monday-Friday, 8:00am-4:30pm, every other weekend

Essential Functions:

  • Schedules field clinicians. Verifies that all requested visits are scheduled. Communicates to supervisors all missed and declined visits.
  • Triages incoming calls; ensures positive customer service experience.
  • Maintains stock of new admission packets.
  • Compiles and transmits HHCAHPS files to outside vendor in coordination with Performance Improvement Nurse.
  • Coordinates volunteer assignments.
  • Verifies physician licensure with Pennsylvania licensing boards.
  • Completes mileage requests.
  • Assists documentation specialist in tracking physician orders and face-to-face encounter documentation.
  • Assumes primary responsibility to order office supplies and storeroom medical supplies.
  • Supports Palliative care staff with coverage for days off.
     

Qualifications

EDUCATION AND EXPERIENCE:
  • High school diploma/GED and over 3 to 5 years of experience required.
