Responsibilities

Under the direction of the professional nurse, the Home Health Aide assists the patient in the home setting with personal care activities and incidental duties which the patient requires. The Home Health Aide ensures the assessment of care is appropriate to the age of patients served: adolescents, adults and geriatrics.

Essential Functions:

• The Home Health Aide provides personal care to adolescent, adult and geriatric patients.
• Assesses the patient's temperature, pulse, respirations and blood pressure to monitor his/her condition.
• Changes and makes bed to maintain cleanliness.
• Assists patients with prescribed exercises.
• Assists patient with application of TED/JOBST stockings.
• Observes patients' condition and reports changes in condition to the Registered Nurse or Supervisor.
• Assists with patient transfers, ambulation and prescribed exercises according to established procedures.
• Documentation of care provided and other pertinent observations.

Grand View Health's Home Care department serves Bucks and Montgomery communities. GVH Home Care strives for excellence by assisting patients with their health needs in the comfort of their home. The Home Care staff is expected to commute within a 15-mile radius of Grand View Hospital. Gas mileage will be reimbursed.
