Grand View Health is a member of the Penn Cancer Network, an affiliate of Penn Medicine Orthopaedic Surgery, and an affiliate of the Penn Trauma Network, partnered with Penn Medicine to provide advanced neurological services, including tele-stroke and tele-neurology care.


Our Licensed Practical Nurse in Home Care provides skilled nursing care in the patient's home and demonstrates competent knowledge and skills to ensure care is appropriate to the adolescent, adult and geriatric patients served. The LPN understands and employs professional nursing skills consistent with standards of Nurse Practice Act of Pennsylvania and Grand View Home Care's policies and procedures. Under supervision of a Registered Nurse, the LPN provides quality care to patients and performs technical skilled care in patient's home.

Job Duties Include:
  • Possessing and practicing appropriate physical assessment skills, including assessment of the adolescent, adult and geriatric patient, as well as the family and home situation.
  • In cooperation with the RN, providing ongoing assessment of patient's condition and nursing needs.
  • Prioritizing patient's problems or needs according to urgency to reflect the physical, psychological, emotional and spiritual aspects of care of the patient.
  • Completing OASIS assessments per Medicare guidelines.
  • Fulfilling on-call responsibilities when assigned.
  • Acting as a case manager to coordinate all home care services. Supervising Home Health Aides.
  • Clarifying and implementing physician orders and nursing care plan according to established standards.
  • Performing medication reconciliation and instructing patient/family in medication use and precautions.
  • Developing and revising an individualized plan of care, and setting mutually acceptable goals with patient/patient's family.
  • Documenting according to departmental standards.
Schedule: 8:00am to 4:30pm, one out of four weekends and two holidays. This role is 80 hours biweekly, exempt and includes mileage reimbursement.


  • Vocational diploma/Licensed Practical Nursing diploma minimum requirement.
  • 1 year to 3 years of nursing experience required.

  • LPN in Pennsylvania – licensed required upon hire.
  • Basic Life Support (BLS) required upon hire
  • Driver's license – PA required upon hire.
  • Automobile insurance coverage required upon hire.
