Under the general direction of the Program Coordinator and Medical Director, provides prenatal nursing and educational services to women enrolled in the Healthy Beginnings Plus (HBP) Program. Demonstrates competent knowledge and skills to ensure proper assessment and care of adolescent and adult patients served. The Nurse Care Coordinator:
  • Interviews potential clients to determine appropriateness of enrollment.
  • Assesses individual medical, educational and social needs and develops a plan of care for each adolescent/adult patient.
  • Coordinates patient care through referrals to and communication with other HBP team members, as well as to outside agencies when needed.
  • Applies principles for teaching the adolescent or adult learner, especially considering patient's reading level and social situation, when providing education materials and/or information to patients, in order to optimize learning.
  • Provides outpatient nursing care and assessment, counseling and educational information when patient presents for clinic visit to evaluate patient's status since previous visit.
  • Provides clear documentation on patient's chart regarding assessments, progress and response to plan of care.



  • Bachelor's degree in Nursing and over 1 year to 3 years of nursing experience in related speciality preferred.

  • Registered Nurse in PA – licensed, upon hire, required.
