Responsibilities

The Nurse Care Coordinator serves in an expanded nursing role to collaborate with patients and their primary care providers to offer a model of care that ensures the delivery of quality, efficient and cost-effective healthcare services. The nurse assesses, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status. The Nurse Care Coordinator integrates evidence-based clinical guidelines, preventative guidelines, protocols and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare for the populations. The RN develops systems of care that monitor patient progress and promote early intervention in acute care situations. The Nurse Care Coordinator works effectively with other members of the healthcare team to optimize interventions. The incumbent manages utilization and practice metrics to further refine the delivery of care model to maximize clinical, quality and fiscal outcomes for identified populations.

Essential Functions:

  • Identifies the targeted population within his/her practice site and risk stratifies all members to prioritize needs and direct interventions.
  • Assesses the healthcare, educational and psychosocial needs of the member/family. Designs an individualized plan of care with the patient and fosters a team approach by working collaboratively with the patient, family, primary care provider and other members of the healthcare team to ensure coordination of services.
  • Implements clinical interventions based on risk stratification and evidence-based clinical guidelines. Collaborates with primary care providers to enhance evidence-based clinical guideline adherence and to promote best practice by initiating/adjusting therapies as directed by the practitioner and providing appropriate follow-up and monitoring as needed. Coordinates appropriate laboratory and diagnostic testing.
  • Coordinates care with appropriate community health and social service resources.
  • Implements systems of care that facilitate close monitoring of high-risk patients to prevent and/or intervene early during acute exacerbations.
  • Continuously evaluates laboratory results, diagnostic tests, utilization patterns and other metrics to monitor quality and efficiency results for assigned population.
  • Maintains required documentation for all care management activities. Collects required data and utilizes this data to adjust the treatment plan when indicated.
  • Works with leadership to continuously evaluate process, identify problems and propose process improvement strategies to enhance care delivery.
  • Reviews the current literature regarding effective engagement and communication strategies, case management strategies and behavior change strategies and incorporates into clinical practice.
  • Incorporates excellent written, verbal and listening communication skills; positive relationship-building skills; and critical analysis skills into case management practice.
  • Utilizes appropriate conflict resolution, assertiveness, negotiation and collaboration skills in facilitating member throughout the healthcare continuum.
  • Maintains confidentiality and follows proper release of information as mandated by the organization.
     
Schedule: Part-time, Monday through Thursday, 8:00am to 4:00pm
 

Qualifications

EDUCATION AND EXPERIENCE:
  • Associate's degree, diploma or certification in Nursing and over 6 months to 1 year of office RN experience required.
  • BSN preferred.

LICENSES AND CERTIFICATIONS:
  • Basic Life Support (BLS) upon hire required.
  • Registered Nurse or Licensed Practical Nurse in PA – licensed, upon hire, required.
