Rehab Facilitator
Part-Time, Days, Benefit Eligible - Monday-Friday, 4 hours per day, flexible time span between 8:30am-5pm Tuesday - Friday, 5 hours per day

Processing referrals for rehabilitative/assistive technology equipment. Responsible to collect all necessary information and documents to qualify services. This includes verify insurance, obtaining Letters of Medical Necessity, authorizations, processing predeterminations, and accurately coding products and accessories in a timely manner.

Essential Functions:

  • Processes referrals for rehabilitative/assistive technology equipment.
  • Verifies insurance and follows insurance procedures as it pertains to rehabilitative/assistive technology equipment.
  • Works with and follows up with physicians, insurances, patients, physical and occupational therapists in order to qualify services.
  • Obtains all necessary documents in order to qualify services. This includes but is not limited to authorizations, process of predeterminations, and letters of medical necessity.
  • Accurately codes products and all accessories.
  • Works with manufacturers and vendors to obtain quotes, cost and coding.
  • Maintains and updated pipeline in a timely manner.
  • Inputs work orders for rehabilitative/assistive technology products.
  • Includes all necessary documents in accordance to insurance and accreditation body requirements to the work order for patient review and signature.
  • Communicates clearly and effectively with referrals, patients, insurance representatives and healthcare professionals.
  • Schedules and conducts monthly pipeline meetings with Rehab Specialist and Manager.
  • Schedules home assessments, deliveries, and equipment service for Rehab Specialist.


  • High school diploma/GED required.

  • Drivers License – PA, upon hire, required.
