Release of Information Representative
Full Time, 8am-4:30pm, Monday-Friday

Responsibilities

The Relase of Information Representative is responsible for processing release of information requests, including patient walk-in requests, telephone requests, physician requests, mail requests, subpoenas and in-house requests on a timely basis. They are to work closely with the Release of Information vendor to ensure all requests are processed in a timely manner. The Release of Information Rep is responsible for patient portal enrollments and related calls regarding patient portal. The Release of Information Representative is also responsible for the Death Registry and the Birth Registry process. Their essential functions include:
  • Processing requests by mail, telephone, fax, email and/or from walk-ins for the release of information from the medical record for use by insurance companies, third-party payers, lawyers, physicians and government agencies.
  • Working in conjunction with the Release of Information vendor to ensure timely processing of all medical record requests.
  • Interacting with copy service representative to provide direction and continuity of workflow.
  • Checking each request for properly executed authorization, signed by patient. Going to nursing units to obtain inpatient authorization as needed in accordance with regulations governing the release of information and following the Grand View Hospital policy for release of information.
  • Exercising judgment against disclosure of confidential information to unauthorized users.
  • Following specified procedures for copying specified portions of medical records. Comparing signatures and typing standard letters to requesting parties following established procedures for release of information, ensuring that each request is answered in some manner.
  • Maintaining working knowledge of HIPAA regulatory guidelines regarding release of the information.
  • Having a working knowledge of the electronic medical record (EMR) systems.
  • Safeguarding patient confidentiality at all times in accordance with hospital policy.
  • Processing all admissions and discharges to predetermined procedure in order to provide coverage for responding to patient care chart requests.
  • Managing the electronic Death Registry process, including the completion of death certificates by medical staff.
  • Managing the electronic Birth Registry process, including the verification of birth certificate information.
  • Maintaining department spreadsheets relating to the duties in the Release of Information Office.
  • Representing the Health Information Management Department of Grand View Hospital in court cases, maintaining a professional demeanor at all times, in response to subpoenas requiring an appearance.
  • Maintaining satisfactory interdepartmental relationships.
  • Performing other duties as assigned for promotion of smooth general workflow, including daily chart assembly/analysis.
  • Operating all equipment within the department as needed in order to be able to handle all types of inquiries made of the department.
  • Ability to deal in a professional manner with patients and outside requestors for health information.
 

Qualifications

EDUCATION AND EXPERIENCE:

  • High school diploma/GED required and over 1 to 3 years of general office experience required.

  • Ability to listen to others required.
  • Demonstrated written communication skills required.
  • Strong verbal communication and listening skills required.
  • Knowledge of medical terminology required.
  • Demonstrated time management and priority-setting skills required.
  • Ability to drive multiple projects to successful completion required.
  • Self-directed required.
  • Possesses familiarity with Microsoft Office (Word, Excel, PowerPoint, etc.) required.
  • Ability to extract pertinent, relevant information forma the patient's medical record required.

LICENSES AND CERTIFICATIONS:
  • HIPAA Privacy Certification within 6 months of hire preferred.
