The Social Worker will provide supportive and informational counseling and discharge planning to adult, pediatric, adolescent and geriatric patients/families as they relate to the patient's emotional, functional, financial and social adjustment to illness, hospitalization and discharge. The goal of the Social Worker is to collaborate with patients, families and healthcare providers to promote health and manage illness. The Social Worker facilitates the use of assessment, coordination and education to achieve quality patient care outcomes while coordinating interdisciplinary, comprehensive and cost-effective care across the continuum.
This is a full-time position and the hours are Monday-Friday, 8:30am-5pm.
Inpatient Social Worker: 2 years of acute care hospital med/surg social work or case management experience preferred. 2 years of healthcare experience required. Knowledge of DC planning, SNF/PCD/LTACH, psych placements and pre-certification processes. Computer proficiency to utilize EMR and case management software. Knowledge of medical terminology and community resources is strongly desired. Strong ability to prioritize and problem solve required, as are critical thinking skills.
Healthy Beginnings: 2 years of acute care hospital med/surg social work or case management experience preferred. 2 years of healthcare experience required. Knowledge of maternal/child psychosocial and medical care issues is desired, as is clinic experience. Counseling skills are required. Computer proficiency to utilize EMR and case management software. Knowledge of medical terminology and community resources is strongly desired. Strong ability to prioritize and problem solve required, as are critical thinking skills.
Medical Access Program/Outpatient Social Work Case Management: 2 years of acute care hospital med/surg social work or case management experience preferred. 2 years of healthcare experience required. Knowledge of DC planning, insurance plans and care coordination across the continuum is required. Computer proficiency to utilize Microsoft Office, EMR and case management software. Knowledge of medical terminology and community resources is strongly desired. Strong ability to prioritize and problem solve required, as are critical thinking skills.
