The Hospice Social Worker will conduct psychosocial bereavement risk and spiritual assessments on all patients and families involved in the Hospice program. The assessment will focus on the emotional, social and grief factors that impact the patient's and family's functional adjustment to the end stage of the disease process. The Social Worker will visit patients to provide medical social services when ordered by the physician as part of the patient's plan of care. The Social Worker assists the physician and other hospice staff in understanding the significant social and emotional factors related to the patient's health problems. The Social Worker will provide counseling to the patient/caregiver and hospice staff and guide patients/caregivers in securing resources needed. The incumbent demonstrates competent knowledge and skill to ensure care is appropriate to patients serviced.
