Volunteer Bereavement Coordinator - Hospice
Full Time, Days - Mon-Fri, 8am-4:30pm with administrative call every 6 weeks

Responsibilities
Under general direction, the individual is responsible for coordinating the delivery of all volunteer and bereavement activities. Candidate may oversee the supportive care team, which is made up of a Bereavement Counselor, Chaplains and Social workers. This individual also assumes clinical supervisory responsibilities in the absence of Hospice Manager and/or Clinical Coordinator. Demonstrates competent knowledge and skills to ensure care is appropriate to the patient.
Essential Functions:

Organizes and supervises the Hospice Volunteer Program to ensure it meets the needs of the Hospice patient and family.

Solicits potential volunteers from the community, interviews and screens candidates, and provides volunteer training and orientation.

Assigns appropriate volunteers for family needs.

Assists with volunteer quarterly News and Notes to keep volunteers abreast of events and department changes.

Maintains a record on each volunteer with pertinent information, including patients assigned and reliability in order to retain reliable volunteers.

Monitors and tracks volunteer hours to ensure the accuracy of hours and cost savings to Hospice as mandated by the conditions of participation.

Organizes and supervises Bereavement Services to Hospice families.

Monitors effectiveness of Bereavement Program, ensuring that it meets the needs of Hospice patient/family through analysis of feedback and participants and via bereavement survey.

Assesses families and identifies for high-risk bereavement.

Develops/revises/updates Bereavement Program policies and procedures.

Communicates with Hospice ID when bereavement counselor is absent, helping to identify families at risk for problems and reporting on bereavement services.

Visits and counsels the bereaved as indicated.

Maintains bereavement statistics and bereavement discharge process.

Plans and implements volunteer education courses and support needs.

Develops and runs annual volunteer training.

Develops ongoing inservices for volunteers to meet their educational and mandatory inservice requirements.

Monitors volunteers' notes to ensure complete and accurate documentation.

Co-signs notes and places on clinical record.

Oversees Bereavement support group.
-
Monitors ongoing Bereavement group.

Develops and facilitates goal-oriented Bereavement group as deemed necessary.

Coordinates and plans for annual memorial service.

Documents according to departmental standards.

Assists with clinical supervision and quality management duties as department needs dictate.

Understands components of Hospice Performance Improvement program and assists in data collection for Hospice to maintain service excellence and comply with standards as set forth by The Joint Commision.

Attends mandatory inservices as set forth by Hospice policy (HOS 420), along with BCLS, and demonstrates knowledge of these concepts in order to meet position requirements.

May oversee Bereavement Counselor, Chaplains and/or Social Workers.
Qualifications
EDUCATION AND EXPERIENCE:

High school diploma/GED and 1 to 3 years of experience in a related role in a hospice or home care setting
or
-
Bachelor's of Social Work and 1 to 3 years of experience as a Social Worker
or
-
Vocational diploma/nursing diploma minimum requirement and 1 to 3 years of experience as a Registered Nurse
or
-
High school diploma/GED and completion of at least one unit of Clinical Pastoral Education (CPE) or equivalent and 1 to 3 years of experience as a Chaplain.
