Home Health Aide - Home Care
Per Diem, Days - Committed - 8am-4:30pm

Responsibilities

Under the direction of the professional nurse, the Home Health Aide assists the patient in the home setting with personal care activities and incidental duties which the patient requires. The Home Health Aide ensures the assessment of care is appropriate to the age of patients served: adolescents, adults and geriatrics.

Essential Functions:
  • The Home Health Aide provides personal care to adolescent, adult and geriatric patients.
  • Assessing the patient's temperature, pulse, respirations and blood pressure to monitor his/her condition.
  • Changing and making bed to maintain cleanliness.
  • Assisting patients with prescribed exercises.
  • Assisting patient with application of TED/JOBST stockings.
  • Observing patients' condition and report changes in condition to the Registered Nurse or supervisor.
  • Assisting with patient transfers, ambulation and prescribed exercises according to established procedures.
  • Documentation of care provided and other pertinent observations.

Grand View Health's Home Care department serves Bucks and Montgomery communities.  Grand View Health Home Care strives for excellence by assisting patients with their health needs in the comfort of their home. The Home Care staff is expected to commute within a 15-mile radius of Grand View Hospital. Gas mileage will be reimbursed.

Schedule: Per diem, between the hours of 8:00am and 4:30pm
 

Qualifications

EDUCATION AND EXPERIENCE:
  • High school diploma/GED minimum.
  • Certified Nursing Assistant (CNA) or must have successful completion of formal training program with required 75 hours of training and 16 hours of supervised practical or clinical training.

LICENSES AND CERTIFICATIONS:
  • Basic Life Support (BLS) upon hire required.
  • Driver's license – PA, upon hire, required.
  • Automobile insurance/registration upon hire required.
