Responsibilities

The Licensed Practical Nurse, LPN, Grand View Medical Practices (GVMP) serves the patients at Grand View practices, collaborating with patients and their primary care providers to ensure the delivery of quality, efficient and cost-effective healthcare services. The Licensed Practical Nurse, LPN, GVMP is accountable for assessing, planning, delivering and evaluating care for patients served and collaborates with other members of the healthcare team to achieve optimal patient outcomes. This Licensed Practical Nurse, LPN, GVMP will support family practices located in Hatfield, Harleysville, Souderton, Chalfont and Quakertown.

Essential Functions:
  • Ensures physicians have information necessary to provide exceptional care.
  • Greets, escorts and interviews patients and prepares exam rooms for physician.
  • Reviews daily incoming office, enters into the EHR for physician reviews and follows up with office physician as necessary.
  • Utilizes computer to gather information, review patient data and schedule appointments.
  • Review of medications and prescription requests, including notification of need for appointment.
  • Coordinates appropriate laboratory and diagnostic testing.

Qualifications

EDUCATION AND EXPERIENCE:
  • Vocational diploma Licensed Practical Nurse training and over 6 months to 1 year in an acute care setting required.
