Responsibilities
The Stroke & Heart Failure Navigator, under the guidance and supervision of the Vice President of Medical Affairs and Telemetry Unit Director, serves as a clinical and program coordinator for the Stroke and Advanced Heart Failure Program, ensuring quality, timeliness and effectiveness of patient care delivery. Supporting the Stroke and Heart Failure Team, the Stroke & Heart Failure Navigator maintains a strong component of patient advocacy with day-to-day review and coordination of multidisciplinary services. Provides formal and informal staff education for professional, technical and ancillary personnel at Grand View Hospital and provides coordination, consultation and/or teaching in the areas of patient education and research for adult and geriatric patient populations, families and community members. Serves as a liaison between the hospital and state and regulatory agencies.
Qualifications
EDUCATION AND EXPERIENCE:- Bachelor's degree in Nursing required.
- Over 1 to 3 years. Minimum of 2 years of clinical nursing experience in neuroscience and/or heart failure nursing required.
- Over 1 to 3 years. Minimum of 2 years of providing community and professional education preferred.
- Over 6 months to 1 year of experience in curriculum development and principles of adult education preferred.
- Master's degree in Nursing preferred.
LICENSES AND CERTIFICATIONS:
- Registered Nurse in PA – licensed, upon hire, required.
- Basic Life Support (BLS) upon hire required.
- Certification by the American Association of Neuroscience within 2 years of hire preferred.
