Documentation and Scheduling Specialist, Hospice
Full Time, 8am-4:30pm, Monday-Friday
Quakertown, PA 18951

Responsibilities

The Documentation and Scheduling Specialist shows competency in all aspects of day-to-day office functioning in Hospice. The Documentation and Scheduling Specialist acts as a liaison between patient and family, hospital personnel and staff. Supports all Hospice staff and performs accounts payable duties as assigned. Has ability to schedule patients using computer software and to perform intake and medical records tasks as assigned. The Documentation and Scheduling Specialist answers phones and processes confidential information.

Essential Functions:
  • Triages phone calls as to importance and/or need and responds to need or transfers to appropriate party.
  • Presents as a first contact for many patients, families or general public. Able to explain Hospice services or refer to appropriate party to answer questions.
  • Gathers referral information and checks it for accuracy.
  • Inputs referrals and admission information into computer and assumes responsibility to see all referrals and admissions are entered accurately in the computer system.
  • Assists in assembly of new charts and scanning and uploading records into EMR following office process to ensure accuracy of consents and forms reqired by governing bodies.
  • Schedules visits for all staff using EMR.
  • Ensures productivity standards are being met.
  • Minimizes milege and travel time.
  • Prepares monthly on-call/weekend calendar.
  • Processes all physican orders and plan of care updates to physicians and facilities. Resubmits unsigned orders in a timely manner.
  • Maintains current census list.
  • Processes medical claims in a timely manner and resubmits rejected claims after corrections have been made.
  • Assists in preparation for month-end close.
  • Assumes responsibility for setting up and obtaining special reports through the necessary computer programs to provide data for decision and program making.
  • Enters payroll information in a timely manner.

Qualifications

EDUCATION AND EXPERIENCE:
  • High school diploma/GED required.
  • Over 6 months of office support duties and computer input, Windows and Excel required.
