Bereavement Counselor, Hospice
Full Time, Days - Mon-Fri, 8am-4:30pm with some weekends & evenings

Under general direction, the Bereavement Counselor is responsible for coordination and delivery of bereavement activities. The Bereavement Counselor demonstrates competent knowledge and skills to ensure bereavement care is appropriate for all ages.

Essential Functions:
  • Organizes and delivers pre- and post-bereavement visits to all hospice patients, families and caregivers as needed to provide supportive counseling, grief therapy and/or crisis intervention.
  • Monitors effectiveness of Bereavement Program in conjunction with the bereavement coordinator.
  • Participates in IDG team evaluation of risk factors of patients, families and caregivers being provided bereavement services.
  • Participates in evaluation of need for revisions in Bereavement Program policies and procedures.
  • Trains, supports and supervises volunteers involved in bereavement services.
  • Contacts bereavement families either by card or phone as per the individualized POC.
  • Provides ongoing counseling to families on an individual basis as deemed necessary. Makes appropriate community referrals as necessary to bereaved families.
  • Monitors Bereavement volunteers as needed when they make phone calls and contact families.
  • Facilitates Bereavement Support groups as directed and develops new groups as need is identified.
  • Maintains records of support groups and attendance, and provides data monthly to direct supervisor and manager.
  • Supports hospice team in coping with multiple losses.
  • Assists with coordinating and planning for annual memorial service with the hospice chaplain.
  • Documents according to department standards using the EMR and uses appropriate reports to tract bereavement activity.
  • Attends necessary in-services to meet standards necessary for Bereavement Counselor.
  • Assists with quality management duties as department needs dictate.
  • Understands components of Hospice Quality Assessment and Performance Improvement (QAPI).
  • Programs and assists in data collection (when directed) for Hospice to maintain excellence and comply with standards set forth by Medicare COPs and The Joint Commission.
  • Provides in-service education to the community and contracted facilities on bereavement as department needs dictate.


  • High school diploma/GED required.
  • Associate's degree in Counseling, Nursing, Social Work or a related field and over 1 to 3 years of hospice experience preferred.

  • Basic Life Support (BLS) within 2 weeks of hire required.
