Responsibilities

The Clinical Care Coordinator is responsible for the coordination of clinical activities of the Wound Care Center including, but not limited to, the use of organizational skills, leadership, planning, implementing, evaluating and providing patient care through the use of hospital and nursing standards.

This is a full-time position. The hours are Monday-Friday, 8am-4:30pm.

Job duties include:
  • Providing case management to a group of patients, providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments and patient/caregiver education related to wound care management.
  • Organizing and prioritizing clinical responsibilities. Providing direct supervision to clinical staff.
  • Collaborating with other healthcare providers, Wound Care Center physicians, Program Director and Medical Director regarding clinic and patient needs.
  • Consistently looking at current processes and procedures, and identifying and implementing areas of improvement while adhering to HIPAA, JCAHO, DOH, hospital and DCS requirements.
  • Engaging in staff development, providing general in-services as well as individual instruction and orientation. Providing staff recognition through positive reinforcement and constructive feedback.
  • Collaborating with Program Director in coordination of the Center's objectives from a budgetary and financial perspective.
  • The Clinical Coordinator is accountable for healing outcomes in the Wound Center, which include clinical practices and patient satisfaction, leading to the success of the Wound Care Center.
  • Attending 40-hour accredited intro to Wound Management and Hyperbaric Oxygen Therapy course in order to function as hyperbaric chamber operator in the event coverage is needed. Maintaining all competencies required.
     

Qualifications

EDUCATION AND EXPERIENCE:
  • Associate's degree in Nursing.
  • RN license required upon hire.
  • BSN preferred
  • 1-3 years in lead or management role, experience in wound care preferred.
  • Wound care certification preferred.
  • BLS required upon hire.
