Grand View Health is a member of the Penn Cancer Network, an affiliate of Penn Medicine Orthopaedic Surgery, and an affiliate of the Penn Trauma Network, partnered with Penn Medicine to provide advanced neurological services, including tele-stroke and tele-neurology care.

Responsibilities

The Joint Replacement Coordinator oversees all operational aspects of the orthopedic service line's Joint Replacement Program. The role of the Joint Center Coordinator encompasses care coordination, patient education and program development.

Joint Replacement Coordinator Essential Functions:
  • Initiates standardized care plan for each elective primary or revision total knee replacement (TKR), total hip replacement (THR), and total shoulder replacement (TSR) procedure as a same-day outpatient procedure or extended outpatient hospital stay.
  • Collaborates with administrative and physician teams to ensure that patients are clinically ready for surgery.
  • Understands and participates in learning the various evolving requirements of insurers and regulators in relation to Total Joint Replacement care.
  • Proactively prevents denials and delays in patient care provision by the insurers.
  • Coordinates discharge planning needs, including, but not limited to, home health services, physician follow-up care, durable medical equipment, medical supplies, healthcare services, outpatient therapy, skilled nursing care and/or assisted living care.
  • Participates in daily multidisciplinary discharge rounds.
  • Identifies and intervenes appropriately in potential quality or risk management issues.
  • Evaluates patient satisfaction and perceptions of quality of care and makes changes as appropriate.

Patient Education:
  • Prepares clients by meeting with them one-to-one pre-procedure to provide education, review clinical needs and provide psychosocial support by evaluating any disparities.
  • Coordinates a pre-surgical class in conjunction with pre-surgical testing to foster a concierge approach to joint replacement.

Program Development:
  • Facilitates Orthopedic Governance meetings with surgeons and multidisciplinary team members. Presents data and reports to Orthopedic Governance Committee and other leadership groups as directed.
  • Coordinates and performs data collection for performance improvement related to disease specific certification.
  • Monitors performance metrics with all activities required to achieve and maintain a disease-specific certifications with The Joint Commission.
  • Drives the program growth using database management and analytical skills to improve outcomes in accordance with national orthopedic standards.
  • Collaborates with hospital marketing staff and orthopedic physicians to publicize the Joint Replacement Center via printed advertisement, speaking engagements, special events, etc., to increase market share.

Qualifications

EDUCATION AND EXPERIENCE:
  • Associate's degree in Nursing and 1-5 years of adult orthopedic nursing experience required.
  • Over 1 to 3 years of hospital-based case management or discharge planning experience preferred.
  • BSN preferred.

SKILLS AND ABILITIES:
  • Possesses familiarity with Microsoft Office (Word, Excel, PowerPoint, etc.).
  • Demonstrated analytical skills required.
  • Demonstrated problem-solving skills required.
  • Ability to make quality, independent decisions required.
  • Effective interpersonal skills required.
  • Ability to drive multiple projects to successful completion required.
  • Excellent organizational skills required.

LICENSES AND CERTIFICATIONS:
  • Registered Nurse in Pennsylvania – licensed, Physician Assistant in PA – licensed or Certified Registered Nurse Practitioner in PA – licensed, upon hire, required.
  • Basic Life Support (BLS) upon hire required.
  • Orthopaedic certification for Registered Nurses, provided within 6 months of hire, required.
